I have a metal back plate on a horizontally mounted bathroom light fixture that has a clearance slot in it - these match the four holes in the plastic box, but I am unsure what screws to mount it with.

No, this box is not rated for usage with a ceiling fan. For a ceiling fan, we recommend using our model 4070CFB-BHL.

Will need a box, a new hole, a way to anchor; no attic space, est. 30ft of wire thru wall and ceiling to existing single switch; will need to change out single switch to double switch. No dimmer. Can I do it and what do I need? This box? And what else?

Answer: 
This box will work for your application as this box is rated for up to a 50 pound fixture. The only other thing we would suggest is a two gang old work box for your double switch - we suggest the B225R-UPC.

Rated 1.0 out of 5.0 by May be ok for some lights but don't use with ceiling fans. I installed one of these in each of 2 bedrooms. They seems to work great for over 2 years. Then one day one of my teenagers returned to the room to fin the ceiling fan dangling down from the ceiling. The plastic box had fatigued and cracked away from the metal bar. the ceiling fan was still spinning. I repaired using a new heavy duty box and a 2x4 between the joists. Fortunately I have access to the attic above. The second one of these in the other bedroom failed the same way months later. I would not have used this for a ceiling fan if the box in the store had indicated so. I believe the manufacturer should be more clear with the usage or the design should be bolstered where the plastic and metal connect. June 7, 2015
Rated 2.0 out of 5.0 by Flawed Product I've had a similar situation. The first time was a light fixture over my dining room table that weighs *maybe* 10lbs. Then another failed. This time it was a pendant light that couldn't weigh more than 5 lbs. Both were installed by the home manufacturer (new construction) and were just over 2 years old. Looking for a replacement solution. January 14, 2016
