if I buy a shower head, can this be used in a shower only

This is made to work for a single outlet setup i.e. ONLY tub or ONLY shower. You would need to buy a separate diverter valve and trim kit to use a dual outlet setup. So yes, this is made to be used as a shower only.

I called American Standard. To use as a diverter you need a different valve, the R522. In that configuration the top handle becomes the 3 position diverter: On/Off, shower, and tub. The bottom is still temperature control. You lose the volume control ability.

Which valve would work for this trim other than American Standard? Also, I have seen in reviews of the trim that the two handles are too close ?

This will only work with American Standard valves. I think the majority of brands are set up that way. I haven't had any issue with the handles being too close. If you install it incorrectly, the bottom lever could rotate too far up and hit the cross handle, but that's easily fixed by adjusting them properly during the install.

Rated 5 out of 5 by from Beautiful Beautiful trim kit really makes the whole shower. Recently renovated a bathroom, using this trim kit along with the matching diverter valve/trim kit, and the shower look gorgeous. Everything is nice, heavy metal, and installed remarkably easy. I definitely expect this to last for years to come (long after we move out!). The design seems to make this very nicely waterproofed, which is of course a good thing.
