Is it possible to use this trim kit to convert an old style 3 valve shower to a one valve and place the back plate transversely (horizontally) to hide the openings from the 3 valves provided that the distance between the two outer valves is < 9 1/4 apart?

No, this can't be done because of the valve that is behind the wall. The 3 handle will not work with the single model.

Does this collection come with a handheld shower head and diverter valve?

No, it does not. It comes only with the standard shower head. The tub spout will include the diverter.
