If you've never removed the seat (it's made to be easily removable!), my guess is you did not 'snap' the bolts into the holes in the seat's bolt flanges before installing. This seat is not installed like a basic seat. You can't just sit the bolts in the flange holes.
You may be able to remedy this while it's installed. Flip open the bolt covers on the plastic flanges and push down on the flange bases until the round holes in them 'snap' into the bolts. If you get them to snap in, then it's fixed. Just close the bolt covers and it should stay on now.
If you cannot get them to snap into each other, then unscrew the bolts underneath and remove the seat, open the bolt covers and then turn it upside down onto a soft surface like a rug. Then insert the bolt heads into the holes in the bottom of the flanges with the bolts sticking up and push hard so they snap into the holes. They should not fall out when you turn the seat over/upright again. Then re-install.
It is designed so you can open the bolt covers and pull the seat off every time you clean the toilet, and snap the seat back down onto the bolts after.

Then I watched the online video mentioned in the installation instructions, and everything went well. It now fits very tight and was easy to install after watching the video, but more so because of the pictures just below the video. Be sure to hand tighten the nuts below the toilet base, and when you can't get them any tighter, use your 1/2" wrench and tighten with about 5-6 turns. You'll know when it's just right. Good Luck. When the lid is placed on the mounting pegs, be sure that the locking "dial" is turned fully counter clockwise, then press the toilet lid down firmly - it should click into place - then turn the dials clockwise to lock the lid in.

Yes, it does make a difference. If you watch the video online, it will show you exactly how to make a perfect installation. The website is listed in the instruction booklet. You will need a 1/2" wrench to remove the not that is screwed on underneath the toilet base. Try that, and I hope it works. Pictures online are shown just below the video.

CUSTOMER REVIEWS

Rated 4.5 out of 5 by 53 reviewers.
Rated 5.0 out of 5.0 by Excellent quality toilet seat This toilet seat was super easy to install and took around 10 minutes for the complete work. The fit was perfect on my round toilet bowl. The construction is made up of glossy enameled wood and feels pretty solid as well as comfortable to sit on. Once installed it can be cleaned easily by removing it from the toilet bowl using the snap off hinge. I would definitely recommend this seat if you are looking for an excellent quality basic toilet seat. April 12, 2016
Rated 5.0 out of 5.0 by Easy to install and clean The seat is well made, solid wood. I like wood because I can sit on the cover and it doesn’t sink in. The finish is smooth and just about as glossy as porcelain. The white color is pure white and a bit whiter than the aged 70 year old American Standard toilet I put it on. It was a snap to install and the instruction brochure was simple and a breeze to follow with good illustrations. (See my pics.) Once installed, you can flip the bolt covers up and simply pull the entire seat right off the bolts to clean the toilet surface, and then drop it right back on again…makes that nasty area around the bolts easy to clean- thus the name! This seat overhangs my toilet front about ¾” which helps to elongate the small size of a round seat- a thoughtful design to improve the opening length of a round seat for sitters. I definitely recommend this product! March 1, 2016
Rated 5.0 out of 5.0 by Easy Install This toilet seat cover is very easy to install. I spent maybe 10 minutes putting it on my toliet bowl. Once the bolts were tightened it stays in place. No slipping out of place. The install videos they show online show you just how easy and simple it is to install. I did read some of the other reviews and feel I should let you know that this is a basic round toilet seat cover. It's not made to be able to use any type of seat cover on it. The top is a little heavy and will slam down on the seat if you drop the lid. Bemis does make a toilet seat cover that is designed to close softly and slowly so as not to slam down and make noise. If you're looking for that type of cover. If not then this cover serves it's purpose. May 19, 2016
Rated 2.0 out of 5.0 by This style lift-off is not secure Bemis offers two different styles of quick release for their lift-off seats. The style that locks with a twist is secure and easy to use. I have two of these. I recently bought this model that has hinged caps that you snap down to lock the seat in place. In use these quickly popped up on their own - "always loose". I replaced it with the other style - problem gone. May 25, 2016
Rated 4.0 out of 5.0 by Excellent replacement seat I have one toilet in my house that has not been replaced. It is an old builder model. it has the old HUGE tank. I don't know why we don't replace it... well, there is or was nothing wrong with it except the seat. The old plastic bolts would loosen on a regular basis and the seat would kind of slip and slide when you sat on it if you were not careful. THIS seat is great! it does not self close, but it also doesn't SLAM down. It fit perfectly in the bolt holes on the toilet base and in the 2 months it has been in use, there have been no slips, slides or self loosening of the seat. This is very nice in the middle of the night. Very easy to install and also cleans well. October 29, 2015
Rated 5.0 out of 5.0 by Love my new Tidy Whitey Toilet Seat Very glad to have received this new toilet seat, as I was due to replace it in the very near future. The quality is better than I expected, and looks perfect in my guest bathroom. The website was very helpful as to the installation instructions. The video and photos onsite were much easier to understand than the little brochure that came with the new toilet seat. I had a little bit of trouble popping up the right hinge, but solved the problem quickly. All of the parts fit perfect, and the hinges are tight thanks to the bolts that also fit perfect. They were really easy to install with only a few twists of the wrench. I would highly recommend this to everyone seeking a new toilet seat. It's also very comfortable to sit on for a while if I decide to do a little reading while on my throne. December 19, 2015
Rated 5.0 out of 5.0 by Great product Very easy to install quiet close and very nice looking May 3, 2016
Rated 1.0 out of 5.0 by Don't buy: Can't use a toilet seat cover!! Reason for rating: You cannot use a toilet seat cover with this product! We've tried two different ones from Target/Kohls and neither fit. I visited Bemis website and they state there: " We design our toilet seats to be stylish and fashionable. Unfortunately, this does not allow for fuzzy covers to fit on the seat. If you are looking for a unique design for your bathroom, we recommend our Sculptured Wood Seats. These seats offer a variety of designs that fit the décor of many different bathrooms." This notice should be placed on the store display to warn consumers. Now I have to buy another seat (it bangs every time it is lifted to use without a seat cover). It won't be a Bemis! March 18, 2016
