Is the difference between lag screw and lag bolt that a lag bolt uses/requires a nut like a regular bolt? Are both screwed through a slightly smaller hole?

A lag screw - or bolt if you prefer - is designated by the American Wood Council National Design Standard used for structural application of wood and timber construction, as a "dowel-type" fastener, meaning that it is bored into and contacts the wood into which it is driven, and is anchored there by the engagement of the screw threads with the wood itself. Pilot holes if used for lag screws, will be slightly smaller in diameter than the screw, to allow engagement of the threads when the screw is installed.
You can also use BOLTS for connecting wood in a structural application. A bolted connection depends solely on bearing of the wood against the bolt body, and uses nuts, washers, etc., to complete the connection. The hole into which the bolt is inserted, is typically 1/32" to 1/16" larger than the bolt diameter. Read Less

Don't want to drill too big a hole for it to grab good but at the same time not too small a whole to force it through.

Answer: 
A good rule of thumb for "lead holes" for lag screws is 70% of the diameter of the screw. This will satisfy the requirements of the American Wood Council National Design Standard, for example.
Thus, for a 1/2" diameter lag screw, ideally use a 11/32" drill bit for the lead hole. I would not recommend a bit diameter more than 3/8".
