What does it mean when it says this ballast can only be used with a disconnect lampholder only

Basically a disconnect lampholder is the same as one of those bi-pin sockets that are normally on fluorescent lamp fixtures.

What is the difference between instant, rapid, and this 'trigger' start ballasts? Does it matter which is used?

If I am correct rapid start is the same as preheat with a starter and instant is like high output and trigger start is basically an automatic starter built into a ballast.

No sorry You will need to find a ballast that says it operates one F40-T 12

Rated 2 out of 5 by from Older-style ballasts Note this is a magnetic ballast rather than an electronic one. The behaviors reported by other reviewers (slow starting, noise, etc.) are much more common with magnetic ballasts, plus they're less energy-efficient. Based on what I've read, magnetic ballasts are rapidly being phased out so my 2¢ would be to go electronic. The cost is the same or only a couple of dollars more, which will probably be made up in later savings. I've rescued two failed fixtures and (OK, at least so far) have had nothing but good results from the new technology.

Rated 3 out of 5 by from Good price, but noisy on startup Product works fine, but has noise on startup (buzz). My wife made me replace it again with another brand because she didn't like the noise. So, if noise is an issue, spend a little more for another one. Otherwise, this one works great.

Rated 2 out of 5 by from Not overly confident. Installed the new ballast and a new tube. Applied power and it wouldn't start. Decided to get to it the next day and experienced the same results. Figured I'd leave it powered on for a while and after a few minutes, it powered up. Since the initial startup issues, the product appears to be functioning, albeit a bit noisy. Though, I must say that the start up issues leave me questioning the quality of the product.
