When it says 24 inches in width, is this the base of it only or does this include the tri-fold doors as it appears doors extend out beyond the base?

This is the width at the widest part of the doors. I haven't measured the base (cabinet only).

Answer: 
If you have the original papers that came with the unit you can write, email or phone the manufacturer (Customer Service) and ask them to send another shelf. It may be at your cost of course.
You may also take the measurements from the existing shelf that came with the unit and go to your local Home Depot and pick up a pine or popular board, finished with a primer coat of paint, cut it to size, paint it Read More

You may also take the measurements from the existing shelf that came with the unit and go to your local Home Depot and pick up a pine or popular board, finished with a primer coat of paint, cut it to size, paint it with a satin or gloss varnish and buy 4 more pegs to hold the board in the cabinet.
Take the measurements you need where you want to place the shelf and mark the placements of the four pegs and drill the holes for the pegs. The size of your drill may make it difficult to drill the holes needed toward the back and you may need to remove the back of the cabinet to drill the holes straight. An alternative to drilling holes for pegs, if you know where you want the additional shelf and you want to make a permanent installation there are metal angles at Home Depot that allows mounting the angle to the shelf and then to the sides of the cabinet, just make sure of your measurements. It's spacious and looked nice while it lasted. but I agree with everyone who commented on the terrible construction (practically paint over cardboard, or perhaps packed sawdust) which disintegrates completely. Hinges on center and left mirrors on ours have fallen apart. Someone commented on "duration" - not good. Reviewer Paul seems to have done well - I'm glad someone did. The price was right, but it really wasn't worth it. Perhaps if the user never, ever gets it wet, but for a bathroom cabinet... seems unlikely. I've never known anyone who didn't splash now and then! Rated 2.0 out of 5.0 by You get what you pay for... I guess at $49 it is not awful, but craftsmanship is not a word that should be applied to this product. As soon as we got it installed, the center door started hitting the right side door upon closing. Apparently, the screws that secure the doors to the frame are set into very cheap particleboard and had already started to pull loose due to the enormous pull of gravity on the small doors. I have already permanently installed it into an existing spot, so I can't take it off and return it or I would. The vinyl woodgrain is peeling at the mitered corners and there are also some scuff marks on the glass that I can't remove. I wish that I had chosen a frameless model, at least there wouldn't be as much cheap particleboard covered with thin woodgrain vinyl. May 22, 2009

While I like the storage option in this unit, do be aware that you really do get what you pay for. This is basically like paper over particleboard. The middle door doesn't shut completely. There are no magnets or fasteners, just the hinges to keep doors closed. It serves it's purpose, but that's about it. Can't comment on durability - that remains to be seen. February 6, 2014

The cabinet has lasted over 7 years and seems to me to be a solid, long lasting product. It fills the need I have for it and functions well, as I expected. I installed it with no assistance except for the use of a level and drill and screwdriver. This cabinet performs as carefree and utilitarian as required. It fits well and looks good right above the basin. August 14, 2013

The person who put this together put the side on upside down so the holes did not match up making the shelf totally unbalanced. My husband had to measure and redrill the peg holes so the shelves would be straight. Total mess! Way too expensive for the cheapness. Don't buy it! July 9, 2010
