Answer: 
It's called 1/4" because that's the inside diameter of the air flow. In other words it fits a 1/4" NPT (National Pipe Thread) pipe which has an inside diameter of 1/4". And yes, the outside diameter of the pipe and thus the pipe thread are quite a bit bigger than 1/4".

Answer: 
This item is called a 1/4" fitting because the fitting 2 measurement is .25 inches.

I bought a set of 13-piece jobsite accessory kit with several 1/4 I/M industrial fittings, but my first attempt to attached the female plug only turns 1/4 turn. The hose is a 3/8" inner diameter 300 psi compressor hose with 1/4" npt male fittings.

Hi Ruffian,
The fitting should screw on with no trouble, check to make sure both pieces are 1/4 inch NPT, and not compression type fittings which are of fine thread.
