Based upon the responses from the reviews, the banana tap has 3 outlets, 2 on one side and 1 on the reverse side.

Rated 5 out of 5 by from PERFECT HEAVY DUTY CORD I love this product, I use it with my 8000 BTU air conditioner. This cord is rated at a high enough rating so that I know there will be no problems. Couldn't ask for a better cord.

Rated 4 out of 5 by from Great cord, just a bit short... Husky has a nice cord here. I like the 14 gauge which is good for up to 15 amps. The three outlet "banana tap" is very convenient also. I like the slim design of the outlet compared to their 25 foot cord where the outlets are arranged three wide. I just wish this cord was 20 (or 25) feet. A 15 foot cord isn't even as long as a car and limits use in a standard garage environment. Great to run into a cabinet to power your electronics too. Keep up the good work Husky and Home Depot!!

I wanted to be able to have enough cord to roll it out of it's area when needed but not too much cord that would tangle up. This cord is rated perfectly for these power tools and has three outlets, one for each sander and one for the swing-arm light I installed. I could not be happier with this product.
