Answer: 
Without a ground connection, you should be using non-grounded outlets, like the 223. Otherwise, you can install a GFCI and use the "NO EQUIPMENT GROUND" stickers that come with it.

WORK WITH THE POWER TURNED OFF
You have to have 2 wires "power transfer wires" running from the switch box to the receptacle box, in addition to the power supply wiring.
One of the wires running from the switch is going to take power TO the switch, and the other will take the power BACK to the receptacle. (understand this)

One of the wires running from the switch is going to take power TO the switch, and the other will take the power BACK to the receptacle. (understand this)
Normally the 2 "transferring" wires will be black and white, wrap electrical tape around each end of the white wire to show future electricians that the white contains power.
In the receptacle box, tie the black power wire to one of the wires (black) going to the switch and a little 6 inch black cut off (pigtail) using a wire nut.
If you have a ground wire running to the switch in addition to the 2 "transfer wires", then do the same with the grounds
Attached the 6 inch ground wire to the ground on the receptacle.
Attach the white power wire to the screw on the side of the receptacle that has the biggest slot.
Attach the 6 inch black wire to the receptacle you want full time power on using the screw that has a small slot.
Attach the return wire (white?) from the switch to the other small slot screw.
Break the metal tab between the 2 small side screws, but NOT on the big slot side
that's it for the receptacle outlet.
On the switch, hook up the ground on the ground screw and either wire can go on either screw.
Install the switch so "OFF" is on the top side (Gravity cannot be allowed to turn on the switch)
