Answer: 
Leviton does not offer a switch in that configuration at the moment. The 1754-ILW is similar, but only has two rocker switches.

Yes. It is designed to control 3 different items. You do have to have the correct wiring between your switch and lights.

Yes, for the vent. No for the heat. I installed a vent for the bathroom that has: a night light, regular light, vent and heat. I placed the heat on a separate switch and the vent on the bottom switch. I had to replace the entire switch within one and a half year. Yes, it will work from a switch perspective and it will handle 15 amps.. That means 1800 watts. I would only use screws for the connections. The current draw from the heater is greater than I would want on the internal spring clips. Because of the heater, your connections need to be "solid gold quality".
Also make sure that you can physically mount it in the electrical box and have a correct switch Read More

You will need 3 hots and 1 neutral from your switch box to your fan to utilize the functions on 3 different switches.
