Is the iron in a chelated form so it can actually be used by the plant?

If it is not chelated it will not be as effective so you have to use much more product. The iron cannot be absorbed readily into the plant.

What is the effective percentage of iron if applied to more than 6,000 square feet?

I understand the contents of one bag are 22% iron with a coverage of "up to" 6,000 square feet. If I spread one bag on 12,000 square feet, does that make the effective iron application at 11%? Said another way, if I spread one bag on 3,000 square feet, does that make the effective iron application at 44%?

We reached out to MFG for more information and below is their response.
“. Always read and follow the label. If apply correctly you will get 22% iron over the 6k sqft. If you have 3k sqft then apply only ½ of the bag and if you have 12k sqft then you need 2 bags. So the answer is yes if you apply one bag over 12k sqft then the amount of iron is half."

Does anyone know what setting I should use to spread this at a rate of 3.5 lbs per 1000 sq ft? I have a Fertilome 40 lb spreader.

If that is true then Dr. Iron Lawn Pellets are an excellent delivery system for long term replenishment of iron to the soil. I've used liquid iron solutions in the past, which give immediate results, but don't provide what the soil really needs. Set your spreader to a lower number because these pellets are small and flow quickly. At this price you don't want to spread it too quickly and not have enough to treat the whole area. August 2, 2016

You can't go wrong using this product at all. You apply it and water and thats it. Give it about two weeks or less to see results. Turns that yellow grass into the greenest.
