why don't these come with a ground screw (or do they)?

Try the pre-fab green ground pigtails. They come with a screw already on the harness. Very fast and easy to use. I prefer the stranded ones, but the HDs near me only stock solid - I order the stranded online and have 'em shipped to the local HD store.

How far a.f.f is code for switches And receps in commercial wiring

Local building codes vary, so check first, but general guidelines for commercial receptacles shall be no lower than 15” measured to the bottom of the box AFF, and switches, thermostats, fan controls, etc, shall be no higher than 48” measured to the top of the box AFF.

what is the distance between the 2 offset mount holes on the back wall? what is the hole diameter?

there are 3 holes shown in pic on back wall. I am referring to the 2 off center holes on right side of back wall

Bri, Thank you for your inquiry on the RACO steel box. Actually, the box has 4 holes in box, not including the raised ground bump. The 4 holes are 1.125” spacing center to center between them, and are 1.5” diagonally across the knockout center to center. The answer to your question so as not to be confusing is 1.125” and the hole diameter is .250”

Rated 4 out of 5 by from Its an inanimate object that does it's job. It would be nice if it came with the ground screws,... Its an inanimate object that does it's job. It would be nice if it came with the ground screws, but those are cheap anyway. On several of them the two mounting tabs (for securing an outlet/switch) were slightly bent and caused my screw to strip and get stuck. When connecting rigid conduit to this box, the connectors stick in far enough that on multiple occasions I had wires contact the connector and short out. If you are stuffing a switch or outlet into it, I'd suggest going with the 4" wide box. Much easier to deal with and you are much less likely to run into these issues or accidentally force a wire to come out. Finally, note that it does NOT work with dimmer switches - the Lutron one I tried to install is too deep and the ground wire attached to the box interferes with it.

Threading done properly for installing electrical switches, outlets, cover plates. No sharp edges, so no need to worry about cuts.
