The Square D QO 200 Amp 40-Space 40-Circuit Convertible Main Breaker Indoor Load Center is UL listed for residential, commercial and industrial power distribution. This load center is built with a shielded, copper bus bar that is plated for reliability. QO load centers are tested and listed only for QO circuit breakers. QO load centers are the "World's Finest" designed with industry leading features for flexible application. QO load centers include features engineered for fast installation and wire savings.

How do I add circuit breakers when all the ground bar and neutral bar screws are filled? Can I put two ground/neutral wires in each screw position?

DO NOT AND NEVER ADD wires to existing screw positions, that is highly illegal and can easily overload the circuit panel, leading to fire and injury.
You can add circuits by using tandem breakers in 15A or 20A. Afterwards, you (or an electrician) can add a suitable ground and neutral bar.
But considering you asking this question, you really need to first consult and hire a certified electrician who Read More

Work safely always with disconnected power, and never add to something that is already full without speaking to an electrician first.

Answer: 
If main breaker handle does not properly operate, replacement main breaker required. QOM2200VH is 200A 2-pole main breaker kit utilized in QO140M200C, Series S, load centers.

There are two different QO breakers for this box, one is twice as much as the other. How come, and which one should I buy?

As the types of breakers were not indicated here, I believe you may be referring to the QO and QO-VH type breakers and what the difference is between them. If this is the case, the QO-VH is rated at 22K AIR and the QO is rated at 10K AIR. Rated 4.0 out of 5.0 by Great price. Good so far. Good service from HD. Great price compared to local electrical supply houses (about half their price). Correct item arrived via UPS. We're installing now - so not yet functional. But the panel was well reviewed elsewhere, so we're not expecting problems. Nice to have the breaker as part of the panel for a backup main shutoff. Since we're upgrading from 100A to 200A, we're wanting a large # circuits (for Arc Fault in bedrooms, woodshop, etc). Hopefully this'll be our last sub panel. Note that (as in the Square D main panel we also bought) the main breaker is VERY stiff and VERY hard to throw - don't know if it'll be easier to throw in time. Lots of knock outs. Easy to install.
