Good Question (I wondered same thing before I went to store)!
Packaging has an important TIP: "Measure the duct opening, not the register or grille"
The 10" x 6" labeling for this register refers to ideal opening in ductwork.
The physical dimensions of this item are:
Outside: 11 13/16" x 7 13/16"
Grill: 9 1/4" x 5 1/2"
Back (part that fits into duct opening): 9 1/2"x 5 1/2"

So the back dimension of register is slightly smaller than the item's labeled size so it will fit in 10" x 6" duct opening.
It was the correct size for my application.
The screw holes were slightly closer together (10 5/8") than my original register (11") but I was able to angle them in to use original holes in ductwork. Read Less

Answer:
Center to center is what it says..... but an easy way to measure holes already in place is to take a tape measure and place the 1" mark at one edge of the first hole..... measure to the SAME edge of the second... And then subtract the 1" that you started with. NOTE... this is only good if both holes are the same size.
I do it every day on my job..... it is easy and it works.

10 in. x 6 in. 2-Way Wall/Ceiling Register is rated 4.6 out of 5 by 29.
Rated 5 out of 5 by from Look good and easy to install They are perfect the only down side I have found is that there was no foam gasket around the inside edge. They won't make an air tight seal to the wall unless you add the gasket yourself. It's not a bad problem but one that only came to light when I went to install them.

Rated 4 out of 5 by from Works Well Good as most other brands but I have had some horrible ones in the past. These worked well but the flap is really stiff, hopefully that will loosen up in time but honestly it may never gets touched that much. Wish any and all makers would make a screw that the paint isn't paper thin and comes off when you install and turns silver.

Rated 4 out of 5 by from Looks nice, tough to install without the right tools We had all rusted brass vent covers throughout the home we just purchased. One of the first things we did was go out & select new ones. These vent covers are a bright white, with a nice sheen to them. The lever is easy to open & close, and they fit perfectly. The only drawback is our vents apparently had slightly smaller screw holes than the included screws. So, we had a tough time screwing them in. Some of the screws have a hex head. If you get one like that, I highly recommend using a hex nut screw driver bit because it's INFINITELY easier than a traditional screw driver.
