Does the adjusting lever go on the right or the left side?

If the adjusting lever is on the right side, isn't that upside down?

When closed, lever is all they way to right, opening vent you go towards left and fully open the lever is to the left. You can install either way; depending on your persepective. One would want the air to be directed to open space versus towards a wall.

What do I measure (length and width) in order to purchase the correct size register? Is it the actual opening of the duct work?

Both length and width; take measurements with you and if in doubt ask one of their trained specialists to assist. Just in case, I usually take the old vent with me to Home Depot.
