Yes, it is an evergreen. If you live in a very cold and icy area there is a spray called "Wilt Pruf" that will protect the boxwood from winter damage. In some areas, icy water damages the leaves of boxwoods but this spray works wonderfully. You just spray it on before winter and the leaves won't get damaged from icy cold water or winds. The spray also helps if you inadvertently plant your boxwood under the overhang of the roof or under the gutters where icy water can drip on them. Try to avoid planting them directly under the roof line for that reason.

How deep should this be planted in the ground when it arrives?

Example: I have looked at the manual and can't figure out what I did wrong.

it says to dig the hole as deep as the root ball and 2 times as wide ,(usually to allow the roots to grow in semiloose dirt not compacted soil. and that the root ball should be planted about a 1/4 higher than surrounding soil so the water doesn't settle around the root stem or base of the plant

How far apart do you plant the shrubs

Depends on how quickly you want them to turn into a solid hedge. I planted 3 feet apart and expect a solid hedge in about 5 years. I would not plant closer than 2 feet.
