Should a hole be drilled into barrel for drainage prior to planting? Or is it recommended to use 'as is'?

Yes, always drill holes so water can drain. For my dwarf fruit trees and even my non-dwarf mulberry tree, I drill at least five to seven 3/4" holes. If it's a Jujube tree or a tree with invasive roots drill 7 holes 3/4" wide and then staple screen over the holes so it won't take up root under the pot.

What is the diameter of the BITTOM of the barrel?

If you are asking about the outsidel diameter... 20"... the bottom of the inside is about 18". The planter is a reasonable value but rather light weight compared to a true whisky/wine barrel. I reinforced the bottom with additional cross 2x2 members.
