Yes it can there will be a gap around it that you will need to have something to cover it. The box will need a rafter to mount too to be able to hold the weight of the ceiling fan.
The cover is to prevent debris from betting into the box during construction. It is removed when installing the fan.

some plastic ceiling box's have the mounting screw go through the box to the rafter under it. Most likely it is not rated to hold a fan. If your able cut out the old box you can put a old work ceiling fan box in it's place.
Jesus Home Depot does sell numerous different 4 inch round box ceiling covers. Some have a knockout in the center of them these are called rough covers. They stock them in 3.5 and 4 inch sizes. If your local Home Depot does not carry them look at Home Depot.com Home Depot sells them in 4 inch size a blank plastic Finish covers, paintable or white finish style plastic covers four inch waterproof covers and four inch rough covers or blank covers.
