All I found was white and stainless steel in my search.
This is a 6-sided cabinet, so the mounting screws - I used 4 total - go two to each back panel that fits flush to the wall. You could use spacers to make up the 1/4" difference, but you might `rack' it and make the mirror hard to close. The answer is `yes' if you put in the right size spacer pair. I had floor-to-ceiling tile, so didn't have this problem. It's really a well-designed unit that's holding up well in service.
Maybe you could custom cut a shelf out of plywood by tracing the one that's included and adding some internal supports.

I would think so only if it's leveled and secured to the wall behind it. The door is rather heavily weighed down by the mirror when opening.
There is room for one more. I made my own. You can get the mounting brackets in hardware.
