Just a matter of opinion, but the cord is not that distracting. Mine is also inside the fireplace, in-place of using real logs. You can always disguise the cord if you prefer. Otherwise, i recommend you shop online for cordless appliances. This particular model was not designed to be cordless.
Not loud at all. Just a somewhat soft muffled sound. We put it in the living room at first, and had no issues. Later in the family room, and either place was just fine. You get used to the soft whirring sound, and it is quite pleasant overall.

It is all one piece, no assembly required. You may have to change the bulb in the back, but it lasts close to 2 years in my case.
It's 110. I use it in a bedroom on my 2nd floor that only has 110 plugs. We've had zero issues with this heater & it isn't a grounded plug head either so it plugs in to any 110 wall socket. Hope this helps!
You can place it anywhere that you want it to be. I stored inside my real fireplace during the summer months, otherwise it can stand alone in your choice of locations. After rereading your question, I would NOT set it inside a faux/wooden fireplace. Most likely would over heat and cause a fire.
