The best way is to clean off the old wax from the bottom of the toilet, position the new wax ring on the toilet bottom, and use your fingers to press and seal the inside and outside edges of the wax ring securely onto the toilet bottom This insures that the wax is truly sealed 100% to the toilet. I learned to do it this way because of an undesirable experience: One time I put the wax seal on the floor, then I set the toilet down onto it. The gasket sealed to the floor but it failed to seal to the toilet bottom ... so the toilet leaked each time it was flushed. Live and learn. :)

NO, you should use a solid spacer on your closet flange to make up the difference. Home Depot sells these. Thicker wax is more likely to leak.
