The adapter (Model ITD35) is designed to attach (be glued) to a 2" drain pipe, but comes with a glue-in adapter for 1-1/2" pipe. The black tail pipe shown in not included, but is part of your own sewer system, with the p-trap added below. They (OS & B) also make a model for PVC (ITD5135) and one for cast iron (ID357).
There needs to be room for the brass tailpiece to penetrate and slide through the seal when you drop in the tub. This adds several inches of length, and doesn't include the P-trap which also needs to be added.
The tail pipe includes extra length so that the freestanding tub can be temporarily installed on short blocks, filled with water and visually checked for leaks.
If you view the video showing installation, the last step before the tub is dropped into place is to lubricate the tail pipe. . . so, yes.

Not usually, unless you cut a hole into the slab and the plumbing is on the underside or imbedded in the slab. This is usually used in new construction or flooring to allow easy drain hookup with limited or no access to the plumbing.
