Installed the 36" on a incline 16.5" from the horizontal plane = 32" cl to cl. This was recommended for assistance getting up from a shower seat.
Don't know about where you are but here its only legal screwing it into underlying studs. Won't hold or take weight if installed otherwise
I do not believe there is a standard height. What I have done many times is to sit on the toilet, and if it is to help you get up and down and for safety, I place it where it is easy to grab. In other words, install it in the most useful and comfortable location for your needs.

Yes easy install.
It can be installed on this surface but if too much pull or weight is applied it could come loose. I recommend as do the instructions that it be screwed into the studs behind the tile and drywall. Safety first.
