It is three pieces. Comes with an easy install wall template, two end pieces and the bar. VERY easy install!
Although the question was asked months ago, it might help someone else. I purchased another one of these today and cut the bar shorter for the space that I wanted. I got a nice smooth cut using a Dremel Ultra-Saw with a metal cutting blade.
Please get anchor screws, self drill into the wall, set your hanging brackets accordingly; enter one end, install the rod, then install other end, simple.
They are included, depending on your surface, you might get heavier ones. We did.

The bar and the pieces on the ends are metal, as are the plates that are screwed into the wall to anchor everything.

All the ones I've seen, including the 24" bar I tested, have side by side mounting screw holes - like those in your picture - with the set screw on the bottom. I can't say for sure that the distance between the holes is universal, but if the old towel bar isn't more than 25 years old, there's a good chance a new set will work in the existing holes. Measure as many times as you need and remember most everything at Home Depot is exchangeable if it doesn't fit.
