No primer needed. I painted over a pre-painted ceiling with one coat and the results were excellent.

I went over drywall and it covered fairly well but it was the ceiling in my garage. If in living space I would have primed the drywall
some stores sell an additive that stops mold from starting. I have used it in bathrooms worked well
Yes it will cover orange peel in the underlying color. But it's not a "leveling " paint. Depending on how bad the orange peel is, it may show as a texture.
Yes but I would suggest a deep nap roller and at least 2 coats at different angles.
