A:  There is a very tiny hex screw that holds the fixture to the separate mounting bracket (which is hidden behind the base of the toilet paper holder. The hex screw is located right at the base of the fixture near the wall, and because the fixture is usually mounted in a position that will hide this screw, you are likely to find this hex screw on the underside of the fixture (i.e., you may need to get on your knees to look up from under the toilet paper holder in order to see this hex screw on the underside of the holder). Once the hex screw is loosened and/or removed using a tiny hex screw wrench, it will come free of the mounting bracket that holds it in place on the wall. Simply unscrew the fasteners that hold the bracket to the wall to remove it, or if you want to leave the holder in place and firm up its' position on the wall, then tighten the mounting screws and/or replace them with better ones for a more secure fit (using, for example molly screws) and then replace the holder on the mounting bracket and tighten the hex screw to secure it in place.
