If I were you I would go to my local Home Depot store. They have a huge selection of covers in stock. The employees at Home Depot are very helpful.
From center to center of the holes they are 2 3/8" apart. The size of the plate is about 4 1/2" X 2 3/4"

Yes, but I would prime it first or use fine sand paper to take the gloss off.
If you have enough drywall I would drill holes and put in plastic wall anchors. If not you could put a small bead of caulk around the hole and push the wall plate into that. Don't forget to put the screws in the holes too. There is enough grip in the holes to hold them in place just fine.
