What does it mean when all 3 lights are on

This option is not present within 6 cases of Tester. But something is wrong. I can assume that Ground and Neutral wires connected directly to each other in receptacle, which is wrong. To check that first of all disconnect AC power by switching off corresponding breaker. Secondly unscrew receptacle and check connections of Ground wire (bare Copper wire should be connected to separate green screw), Neutral White wire should be connected to one side screw with wider female connector in outlet. Ground and Neutral wires should not be connected to each other in receptacle. Each of these wires going separately to Power Load box and connected to Ground and Neutral strips in the box. ONLY in a box Neutral and Ground strips connected to each other. If you will found connection between Ground and Neutral wires inside the receptacle, disconnect them and connect Ground wire to green ground screw and Neutral wire to neutral side screw. Turn power on and check with Tester again. If all wires connected properly you will see two Amber lights ON and Red light OFF. If you see different situation call to electrician for assistance.

what does the open neutral and open ground mean

It means that these wires either have bad contacts to screws inside the receptacle box, or Neutral or Ground wires are not connected to Power Load Box in a house. To check quality of connections inside the receptacle box, turn off corresponding breaker and convince that no AC power in receptacle using contactless tester or Voltmeter. Unscrew and pull out receptacle from the outlet box. Check for loosen screws and tight them. Check for oxidized or corroded wires and clean them with sand paper. Check the screw plates in receptacle. If you see corroded plates clean them with knife or replace receptacle (just about $1). Connect all wires back, tight all screws, put receptacle back to outlet box and check AC power again with Tester. If all wires connected properly, you will see two Amber lights ON and Red light OFF. If you see something different call to electrician for assistance.
