What is the diameter of the insert? I have a 3 1/2 insert that needs to be replaced.

Yes you do have to ask a building code inspector rather than me mine is installed above a dry crawlspace but in a hardwood floor and it comes hill down to does your area flood or never flood any outlet that you install in the floor Has to be in Safe

Mine was too and I wanted to replace the plastic outlet the contractor installed. This outlet is every bit as nice and quality made as it looks. The only problem I had was the outlet cover brass screws, I had to shorten the 12 o'clock and 6 o'clock screws by grinding them a bit.

Can this be used with GFCI and what is the depth.

You may need a special cover plate. Should check with the manufacturer or check at the store. Depth is adjustable.

Difficult installation
The plate is beautiful when installed, but the metal electric box is very difficult to install. The required opening in the floor is non standard, non rectangular, and very specific. If you get it wrong: too narrow the box wont fit; 1/4” too wide the box wont mount properly; if the corners are cut square they will show outside the circular plate. If this is replacing an existing box, getting the romex wires into the ill placed narrow knockouts is a challenge, to put it mildly. Not for weekend warriors.

I bought two of these floor boxes for a home theater projector that is positioned in the middle of our living room. We mounted one for power and the other for audio and data. After a bit of carpentry (on the hardwood floor) and some wiring, it worked like a charm. It is made of high quality material and is pleasing to the eye. The brass color has a brushed polished look to it. The lid closes somewhat tamper-proof. It requires a flat-tip tool to open it. You can not open it with your fingers when it is completely shut. You can adjust the recessed height of the interior jack to accommodate various types of plugs. You can buy keystone plates that allow you to mount various connectors such as a 3.5 mm audio jack and a networking jack, as I did. It fits perfect. The rubber fittings on the lid are removable so that you may pass the cable through it when the lid is closed. Time will tell how quick they wear out over time. For the moment, I keep the lid partially open when I have cables plugged in. I've seen similar floor boxes online and most cost twice as much, if not more. For the cost, this was my best choice. And, they had it in stock at the local Home Depot store. I would recommend them to anyone.

Box looks good and works great
Install was pretty simple - cut a hole in the dimensions they recommend - run wire, connect wire to box - then drop into floor. Only issue I'm having - I think the brass screws that hold the plate down are too short for what I'm doing. - My subfloor is plywood, with fiber board on top and my plate is on carpet. - I think the carpet makes the brass screw too short to get to the plywood and one of the 4 screws that came with would not connect up. - I'll find longer versions of the brass screw and retry, but for now - 3 are solidly attached, plate looks good and works great. - I'd recommend it definitely!

Great choice for a floor outlet
This is a great floor outlet. It looks nice on wooden floors. A plus is the plug goes down into the box, allowing furniture to be placed directly over it. Installation was easy. Only drawback, and it's only a small one, is the door is a little hard to open. Not a big problem as you probably won't be opening and closing it often.

Box and cover are superb. Duplex outlet that is included is a little on the cheap side, particularly for the direct wiring holes in the back. There are no release slots.

Easy to install. Like most jobs preparation took more time. Crawling in the crawl space to find the spot between floor joist and beams. Wiring Simple if you a accustomed to basic household wiring.
