Yes, I have used it on a Westinghouse tankless water heater for 3 or 4 years now.
Doubtful - this is still a hot water heater; just no tank, so the exhaust is not really different than a traditional one. The length of the exhaust run should be enough for most water to condense out of the exhaust. You need to drill a hole in the outer tube and add a fitting for a drain hose (1/4" clear plastic flex tubing) but i do not see any water in that so guessing the exhaust has little. Therefore I do not think it will frost closed.
The length of this unit cannot be extended. However, there should be no reason to extend it. The 3" PVC pipe that connects this unit to the heating unit is what you cut to fit the distance between the two. Refer to your heater's owners manual for maximum allowable distances.
