Yes - the nut and washer do not have associated scanning codes. You will have to purchase an additional washer/nut to fasten the other end.

What is the proper drill bit size for these?

It would be helpful to include that info in the specs. Thx.

The drill bit is the same as the bolt diameter (3/8, 1/2, 5/8). Bolt should fit neatly and snuggly in a cleanly drilled hole.

Yes, it can be installed before you pour concrete, but why not just use standard anchors? There's no need for a "retrofit" bolt.
