I'm at 7000 ft. elevation and the stove works fine. My guess is you wouldn't have any trouble unless you were over 10,000 ft. elevation. And even then it still might work just fine. The problem if any you would run into is an unclean burn - more yellow flame than a clean blue flame because of the lower oxygen levels at high elevation.
The range can be used outdoors. If the outdoor kitchen is enclosed and wild-life cannot access then it is not a problem. However, if it is an open kitchen we would not recommend using this range outdoors.

Yes, the Unique 20" Classic Range will work at high altitudes of 6000+ feet, You will simply need a licensed gas tech to adjust the pressure of the orifices.
Yes it includes the handles. No it will stick out. Mine sits next to my refrigerator and they are even so you don't even really notice that it sticks out further from the counter. You do need to be sure to leave at least 1 to 1 1/2" from the wall for the propane fitting.
