Yes all the wires are permanently attached and long enough to fit into any 4' x 4 bulb fixture.

The ballast comes with more than adequate lenth of wires to connect to the existing wires in the fixture.

Answer: 
Yes it has the wires attached to the ballast. They're folded up in the package and are quite long. Ordering online you'll be fine. You will need wire nuts.

I have a 4 bulb fixture using (2) Philips Advance RELB-2S40-N 2 bulb ballasts. Can this be used to replace both these ballasts? RELB-2S40-N specification sheet attached.

It’s not a direct replacement going to a 4 lamp T8 instant start ballast replacement from a fixture wired for two 2 lamp t12 rapid start ballasts, as you will need to re-wire the fixture from a (2 lead per lamp) rapid start wiring configuration to a (single lead) instant start wiring configuration. You also need to change the lamp sockets from ones designed for T12 lamps to lamp sockets designed for T8 Read More

Rated 5.0 out of 5.0 by T12 to T8 Upgrade I have two four-lamp fixtures in the kitchen. Each has two magnetic ballasts. A lamp went out and I was out of spares. I decided right away to get ahead of the T12 phase out and do a retro-fit on the existing fixtures. I installed one electronic ballast in each fixture paying attention to wiring as in this case I was converting from Rapid Start to Instant Start. I replaced all the lamps with Phillips T8 Kitchen and Bath mainly for a warmer color. The result is more and better quality light, quieter and cooler. The energy savings is a bonus. By utilizing the existing fixtures in place I saved at least $50 and time. The lights in my kitchen tend to be on for long stretches of time. With installations that have a lot of on-off cycles, consider a Rapid Start Ballast for longer lamp life. June 25, 2014
Rated 5.0 out of 5.0 by Not a Philips Ambistar ballast when ordering online for store pickup The ballast is listed online as a Philips Ambistar. Yet when you pick up in store the part is made by GE. If you look at a list of ballasts check the item number and you will see the GE and Philips one are the same. Other than different brand, the ballast has a some hum to it, but not as noticeable as the original 15 year old ballast. Easy enough to install, pay attention to the wiring diagram. I could not get it to work with just two lamps. October 21, 2014

T8 32-Watt Soft White (3000K) bulbs and significantly improved the light quantity & quality from the two 4-bulb fixtures in my home office... Other benefits: I eliminated the buzz I was getting from the older T12 40-watt bulbs and ballasts... and the energy savings should easily pay for the new ballasts & bulbs within 2 years. Installation was straightforward by just following the diagram on the new ballast. July 1, 2014

Rated 5.0 out of 5.0 by Easy to install and worked better than factory original I had and expensive overhead kitchen fixture that had 2 ballasts installed, one for each pair of bulbs, when one went out due to age I figured the other wasn't going to be far behind. When I got to the HD I found that it would actually be less expensive to buy a single ballast that would handle four tubes and trash both the old ballast, I did that and was very pleased with the finished project.
