A:  You would need to reach out to your local store for installation cost. The door does comes prehung.

A:  The frame is primed wood with a sill key at the bottom. You can special order at your local store with a pvc jamb option.

A:  Will not work on the height... since actual unit height is 81-5/8". Width... You can special order a door with 14" sidelites with a unit width of 67.5". You can then have the door cutdown 1/2" to 5/8" to accomodate the height.
