Does this come with everything you'd need to mount to wall or is it just the wire shelf itself, hardware sold separate?

It comes with everything, but you will need tie wire to secure the bracket to the shelf as the bracket cups from the top, not the bottom.

How much weight will the shelves hold for example clothes

If you install as is, not much. If you secure the bracket to the shelf with tie wire, you should be able to hang jeans, sweaters, coats, etc.

Where should the support brackets be placed as directions for it aren’t feasible for 48”?

Directions say to place support bracket no more than 3” from edge and no more than 36” apart. That only cover 42” of the 48” that make the shelf.
