A:  I bought mine about 6 months ago with no problems, I would return it and get a new one.

A:  We had the same problem. Poorly manufactured ring - the groove's not deep enough for the set screw to hold it securely. After about the third time the ring fell out, I noticed that there's a set screw in the back of the wall mount that holds the ring to the wall mount. I took out the set screw, applied some LockTite thread sealer, and screwed it back in tight enough to make the ring's swinging a little tighter. The ring hasn't fallen out yet. Good Luck!

The set screw is located on the underside of the decorative hub. If you cannot tighten the screw, your mounting hardware may be damaged.
