Q:The maximum adjustable support range appears to be less than 2". If the terrain drop/rise over the distance of the gate swing arc is greater than 2", will this still be useful? Unless I'm missing something, will a 4" terrain differential result in the wheel will either binding (preventing full opening or closing of the gate, depending on the direction rise/drop) or the wheel hanging above the grade (not providing intended support). Are there other models that work for a terrain drop/rise of 4" or more?

Q:Will these work on 5 foot wood gate to keep it from sagging? I have a 10 foot wide double gate (2x 5' sections) that open into my back yard. Will they roll well on a grass surface.

A: They would work for the weight, but probably not on the grass surface..You'd probably want a bigger wheel, and preferably, one with tread...
You could of course buy them and just try rolling them by hand across the grass to see for yourself...
You can also put a board down staked/nailed into the grass/dirt that these would then roll on, giving them the hard surface they need..

A: I used these on our double door driver way entry gate. Each door is 144x50 http://www.homedepot.com/p/Ranch-Master-144-in-x-50-in-Wire-Filled-Tube-Gate-40132127/100319503. These did fine. We used a bracket to hold the wheel in place.
