The product includes a backplate that has knock-outs specifically configured to mount to a standard junction box. For safety and building code compliance, a junction box should be used.
Is the glass shade removable? How is it held in place?
The shade is removable. There is a retaining ring inside the shade that tightens to the socket.

Hello- Yes, this is an older product. The detail specification texts haven't been updated to reflect the recent legislation. An LED bulb will work perfectly in this fixture.

Is the light arm 5” long? I need the light bulb itself to get beyond a 4 1/2” medicine cabinet.

No. Use a pancake junction box if over a stud.
Can this be used with a surface mount medicine cabinet that is 4.5 inches deep?
