one picture shows it with a tray on top. Another picture shows it with a flat surface on top--with no tray. Which is correct?

Again, I am not sure, but I really do not think so. You may want to contact the manufacturer or the store that actually sent it to you through Home Depot!
I am not sure. I seen this question before and I really do not think you can order just 1 drawer. You would have to purchase another set, as shown. You might want to call the manufacturer~ Sorry. not much help!

Can I place heavy beads in all the drawers or will it topple over?
This seems to be really sturdy, but I would recommend putting your heavier items toward the bottom, just for safety. It is not worth getting hurt over if it did topple over. Treat it like you would an upright filing cabinet!
You are able to place beads, however, please keep in mind of the drawers weight capacity. Capacity per drawer: 3.3 lbs.

I would suggest putting anything heavy from the bottom up. You don't want to make this top heavy.
