When you are flipping the shower head to another setting does the water spray the ceiling? Does all the settings continue to spray or just the one pointing down?

How big is the actual spray surface, and does anyone have problems with it spraying out of the shower? I have another model of Kohler shower head that is too big for my shower; it leaks out of the door. I was looking for a smaller and adjustable head, but the reported diameter of this one is the same as my current one (5-3/8 inch).
The spray openings (holes) measure 3-1/2 inches wide, centered on the shower head. When I encounter spray coming sideways, it is always due to lime build up on the spray openings. I just use my finger nail to dislodge the lime on each hole that might be affected. That does the trick so I don't have to use any harsh cleaning products to remove the lime build up. FYI, the outer ring of this shower head is just a frame for the head itself that turns with different sprays from each of the 2 round sides and each of the 2 rim sides = 4 different sprays. The 5-3/8 inch is measured side to side on the rim holding the spray head that swivels. Hope I explained this effectively; I love this shower head and would buy it again.
This is the best shower head we have ever had. The spray is large drops and contained on the body. All four sides are actually very similar (and that's good). This inquiry is a good reminder for me as I was going to order another one for the guest bath/shower as the grand kids and their parents are coming to stay with us for Thanksgiving and that shower head sucks!
