I bought mine in the store. The bag and charger were sealed in a plastic bag. The blade was oddly in the saw. The batteries were also oddly attached to the saw. I was concerned this was repackaged. The books weren't packaged in plastic either.

Have ordered large amount of tools from HD & what I have found is that large items like saws get shipped without being put inside another box & sometimes I have received the box damaged or retapped as you you said items might have came out and where out back in by the transport company & resealed, however fed ex ups puts a label to call them it the item was damaged. I have continued to purchase from HD because of there return policy, it’s awesome return it for full refund and not satisfied.

Thank you for your question on the XSH06PT dust extracting attachment. The part number is 911128-8 P.H.SCREW M4X16. This attachment attaches to the saw to allow connection to a vacuum hose for dust extraction.
The screw is 15mm long and an m4-0.7 diameter and thread pitch. The outside diameter of the dust attachment is 40mm or close to 1 1/2” and the inside diameter is around 34-35mm or l 3/8”. I have a Rockler Flexiport hose and the 1 1/2” coupler port that comes with it fits on the saw dust attachment. I’ve also used the same port coupler on my Festool track saw so it is probably a common size. I would contact Makita about the missing screw. They may send you one for free. I hope this helps.
