      A:  Yes, in theory 2 hooks together should be able to carry 40 lbs. However, it is not advisable to hang a 40 lb weight on two 20 lb hooks because if one hook comes loose for any reason, the second hook may immediately fail. For hanging a 40 lb object such as a ladder, either use 3 or more 20 lb hooks or use 2 or more 40 lb hooks.

          A:  Yes, they would need anchors (or expanders) on concrete or drywall. As the hook is 5/16 inch in diameter, you should get anchors suitable for 5/16 inch screws.

                I own about a dozen of these and find them well suited to a number of different applications.  In my garage, I use them to hang tree stands and other small objects.  In my basement, they support storage of crown molding and other small dimensions of wood.  They work well and definitely support the 20 pound working load the item claims--as long as they are appropriately mounted into a stud.  These are affordably priced and they do their job; however, the surface coating of mine was removed after repeated use for hanging metal objects over the course of a year.  It's not a problem to me, but as you can see in the pictures it is noticeable.  Like other reviewers have noted, I don't use this to support a ladder either.  I believe there are better products to support those with my choice being these (http://www.homedepot.com/p/Everbilt-10-3-4-in-Black-Heavy-Duty-Wall-Mounted-Ladder-Hanger-18012/202305456)

                  Very useful, I've been using these for over 30 years. I always run out. You can hang just about anything on them, under 20-30 lbs. But its best if you screw on solid wood, like a ceiling or wall beam. If you use on drywall, bathroom tile or thin wood make sure to use anchors & gorilla glue or some type of chalking glue on the threads & hole to strenthen the hook.
