AnswerMine did the same thing! It is kind of bad design, as round bar, and yes, hole is round but not good fit because of hole not ending with flat inner wall! When you get new holder try some scotch tape on end on each side leave a little over edge to fold over. just an idea, worked for me!

Reasons why I am unsatisfied with this item…
1. Not on 24 inch centers as advertised. Is 24.50 inches and I do not want to cut the bar down. I was hoping this would match an existing towel rail at 24 inches.
2. Mounting brackets have “UP” the wrong way.
3. I had to grind the back of one of the ceramic housing for the
bracket to fit into the cavity.
4. There is no adjustment up or down to level the towel rail. Make sure you are accurate with the four mounting holes. The manufacturer could have provided vertical slots in the bracket. How about providing one vertical and one horizontal slot on each bracket, you mount it with two holes before the next two? See also 6 below..
5. No picture of how it is mounted on the outside of the box. I politely did not rip open the box in the store to see the mounting procedure, I should have!!
6. The most annoying is the template is not centered vertically, more likely with the bottom hole on the bracket. I wanted to line this up with an electrical outlet. Yes I am picky!!
In my opinion this is not easy to install.

A quick note - if your package comes with two different types of anchors, do not use the self threading white ones.; the included screws are too large for those and they will twist right of out the wall, leaving you with a couple of large holes to fill. Use the standard drywall anchors. (See photos).
Other than that, installation was simple, the included paper template eliminates the need for precise measurements. Tape it to the wall, mark the holes,install the anchors. Docked one star because the package included both types of anchors - I chose the wrong ones and it took much longer than it should have to install.

Mounting hardware doesn't fit
I should have read the reviews prior to purchase, but throughout the years I have bought and installed many towel bars. This one is so poorly made with a lack of quality control that Home Depot should discontinue this immediately. The mounting hardware was too large to fit into the porcelain sleeve. And of course, by the time I figured that out, I was almost finished with the project. Do NOT buy this.

One of the holes that the bar is supposed to...
One of the holes that the bar is supposed to go into was not as deep as it should have been, so the bar is not as stable as it should be. Also, the round porcelain pieces that attach to the wall did not attach flush, so there is a gap between the porcelain and wall. Overall poor product quality.

I bought 3 of these and for only one reason. My daughter accidentally dislodged one of the end pieces that the towel rack is inserted into that is attached to the wall and it fell to the floor and the ceramic broke into pieces. So I bought a new towel rack to replace only the one broken piece. Guess what, none of the pieces were manufactured correctly because they would not slide over the metal bracket that secures them to the wall. In short, Glacier Bay has zero quality assurance in place to avoid this issue. I bought and returned 3 towel racks because of this defect. I then discovered that the toilet paper devices look the same and I bought 5 of them thinking I’d get lucky and find one that was manufactured correctly. Thank god that the first one that I opened worked. I then returned the other 4 to HD. What a headache with all the purchases and returns and driving back and forth to HD.

Replacement for the same item purchased 11yrs ago. The bar is now thinner/lighter metal, and longer than its predecessor. The arms that hold the bar don't slide onto their wall brackets very well. Brackets are wider than the space on the back of the arms. Forcing the arms onto the brackets will pull the wall anchors out of the drywall. So I had to bend the brackets, which wasn't ideal as it created a space behind the arms and the wall. I ended up reinstalling the former brackets, and cut down the length of the bar to get it to fit the existing anchor holes. The divots in the arms that hold the bar aren't as deep as the older version, so even using the older (shorter) bar, the arms were too far apart to slide onto the brackets.
