It depends(I know, crummy answer). I mounted mine at 60", which is the height of the towel cabinets you can buy for over the toilet. But if you want more room,or want a second shelf; take 1/2 the length of the towells you want to hang, and add 12" to it.

The short answer is that it will hold more weight than you should put on it. Ultimately it will depend on your wall substrate. If you are attaching through sheetrock using expandable anchors, it will support less. If attaching to studs, or other wood substrate, it will support more.
I don't see why not. I would be sure to run a bead of clear silicone caulk around the top posts that cover the bracket holding it to wall. That would keep any water from leaking into the wall.
