adjustable has a very wide range of fit. Fixed has a tiny range. I can definitely recommend adjustable.
I would say more like 50 inches. As I recall, the tubes slid together pretty far and the longest tube is about 40 inches.
Do you mean, ABOVE the edge of the curtain? That would entirely depend on how far from the top edge of your liner the grommets were. That said, mine has worked fine, but be sure you put your liner on the longer of the two arms, and then that's the side that goes inside the tub.
