A: I would say just a little larger, you can slip a pressure treated 4x4 into the sleeve easily. I had to replace a 4x4 recently, just pulled the bad one out and slipped the new back in, no problem..
A: I have purchased the 4x4x30 Post Protectors and installed them over a 3-1/2"x3-1/2" wooden post and then inserted them in the ground and finished with concrete around them. I checked today, 07-10-22, and the inside measurement is 3-1/2"x3-1/2".

A: The usefulness of a sleeve for replacement is not the intention but is true. A post 2 feet or more down has never required concrete to be rock stable and IMO/experience, dirt fill is 100% better. A gate post could warp due to the weight too.
A: Yes, you can remove your post and slip a new one if needed. I let my sleeves extend 6" above ground to help keep the dirt out and also better protection. Thanks Don L
