Does it check the continuity of the outlet when there is no power?

Answer

No, this item does not have any internal power source. It checks the wiring configuration itself. For example if you hooked up a grounded outlet and getting the single yellow light in middle, then the ground is not connected properly. So checks continuity, but outlet needs power.

What does it mean when it indicates an open ground when ground wire is securely attached?

Answer

It means it is not connected at some point in the circuit, or it's broken somewhere along the way. This tests the circuit back to the panel, not just the outlet it is plugged into. You have a bad ground. I recommend you check with an electrician.
