QUIKRETE® Walk Maker® Building Form are designed where the mold will require one 80 lb bag of QUIKRETE® Concrete Mix (No. 1101). Each form will cover a nominal area of approximately 4 square feet at 2 inches thick.
QUIKRETE® Walk Maker building forms are designed to contain one 80-pound bag of mixed concrete per application. You can use either a gravel-containing concrete like Crack Resistant Concrete Mix or a product without gravel like Sand/Topping Mix for greater finish ability.
One 60 pound bay of Quickrete
Buy one and just fill half if there is enough room for the whole thing to fit. If not, fill 1/2 of it on a board, let set and dry and then just take the pieces you need off and place by hand around the post. We didn't fill our gaps in.

I have never done that but after over 15+ years of using these forms I would bet you could. I have these around most of my house and have spent little time preparing the soil underneath either type of soil or leveling it.They are most forgiving and after a couple of years blend well with my country home. I have poured these over sand , clay and rocks etc.. I am happy with the results. If you do try this I would consider coating the old walk with a concrete adhesive. Pictures below are of some of my walkway. You will need to find something to put at the edge I expect. I use small 3/8" Pea stone to fill the cracks. Sorry I could not be more help but I would give a shot,

This mild winter wasn't a good test, but last winter was much colder with sub-zero nights. My Walk Maker weathered both winters just fine propped against a tree. It is made of a heavy grade plastic, and with no depressions where water can accumulate and freeze, it's hard to see a situation where cold could damage it.
