The hole pattern is both 3" and 4". The cover it's self is 5" around.

These knockouts are for boxes that are smaller than the pre-drilled holes. You need to drill them out and drill the outside for a counter flush screw configuration. Then you install the plugs over the original holes.

On Jan 23, 2019 they provided this answer to another question. "This cover has two sets of holes on it. One set is 3.5 in. apart and the other is 2.75 in. apart. So depending on how your are set up, this may work." I hate to assume but that's what I'm going to do and say that this is probably correct and the 3.75" was a typo.

will this cover also fit on a 3 in ceiling box?

This cover has two sets of holes on it. One set is 3.5 in. apart and the other is 2.75 in. apart. So depending on how your are set up, this may work.

This item takes the standard screw at the top and bottom of a standard round cover plate. I am not sure of actual size.

This cover is exactly what I needed to cover up an old ceiling box. The cover has 2 sets of screw holes, one that's 3 1/2" apart and another that's 2 3/4" apart. It comes with 2 screws and 2 plastic plugs. I used the holes that are 2 3/4" apart so had to punch out those holes with a 1/8" drill bit. I then used the plastic plugs to cover the other holes. Since the plugs make the holes almost invisible, it would have been nice if they had drilled out both sets of screw holes. But I suspect in most applications, you would be using the screw holes that are spaced 3 1/2" apart.

These ceiling box covers are great to cover up boxes that are not being used or even just holes. Very cheap and very easy to install.

We covered the area from where were removed a light fixture from our hallway ceiling. I didn't want to cancel the power and close the ceiling fully-- in case we decide to have another fixture there in the future. It looks inconspicuous and if we ever decide to have a fixture there again, we wont have to break the ceiling again.

Worked but had to make it work
Has two sets of holes. One 2.75” apart and one set 3.5” inches apart. The 3.5” holes are predrilled. The 2.75” are not. I used the 2.75 holes so i had to drill these out prior to using. It wouldve been nice if they were predrilled. Also, the 3.5 holes are counter sunk. I had to also counter sink the 2.75 holes. After doing all this, the cover works! I had a hard time finding a cover that would fit. My home was built in 1965. This comes with 2 screws and 2 plastic plugs. The screws work in both sets of holes and I used the plugs in the 3.5” holes

If you bought non-metallic for purposes of drilling a hole...
If you bought non-metallic for purposes of drilling a hole in the middle make sure you secure on a flat surface and drill on the underside or it will crack and break in half.
